Database contains only original XML code and only if modification installed via Extension Installer. When put into "system" folder it goes directly into storage/modification folder, which is quite different.

Name your modification file alphabetically so that it succeeds the desired modification. I mean name and code.

One of the extensions calls controller improperly. There's no ControllerModule class in default OpenCart.

Yep, the problem with the default one is that it generates the XML every time with no caching and sometimes Google will refuse to wait for this.
It also creates a hard-coded XML instead of using DOMDocument, which is sooooooooo lame. Let alone lack of any customization. Yuck!

There's a default Sitemap extension, but it sucks. I would like recommend you my Smart Sitemap extension, which gives you full control over the sitemap and was tested at least with 100K products. Should handle 200K without any issues as well.
