Arjan - usually your webhost will not allow a PHP memory limit of 512mb, so even though it may be that in your php.ini, it is being over-ridden by another source. You may need to use less categories, or upgrade your server to use this tool.

The Auto Import can deal with all four of those issues, it does not require a custom CSV file, it will work with the CSV you have as long as it is consistent and logical.

Try turning display errors to Off in your admin System -> Settings page. PHP errors that get printed in your JSON outputs are hard to catch but will break all your JSON responses.

I've seen this error a few times. Make sure your user has write permissions on the image folder and the image cache folder. Also make sure the image files you are uploading are valid images (not animated gifs or something).
