Try to install in a subfolder 3.0.3.1 and see if that extension work.

Possible issue with another installed extension.

You can open ocmod extension....and you can add the code directly in files.

Cache folder isn't not important, will be regenerated after

You need to move all content from image/data to image/catalog and on database image path should be catalog/something.jpg or if you use separate folders catalog/some_folder/something.jpg

1. Go to Extensions-->Themes and check if Default theme is installed
2. Go to Settings, edit you store, select Default theme and Save.

And should work.
