I've found similar sorts of problems with "cut & paste" of coding, with extra lines of space being added. This is primarily due to Windows using a carriage return AND a line feed, while Linux and Mac just use a carriage return. Text editors in Windows still behave like typewriters!

Agreed. But I don't like to assume anything, after all, he did say that he added "shipping_cost.php". The real question here is, how did he add it?

Did he? If you are referring to-

I got to Admin->Configuration->User->User Group and added "shipping_cost.php" to allow to access and modify,
There is no mention of editing the "Top Administrator".

Go to Admin->Configuration->Users->User Group, there should be an entry there for "Top Administration". Click the update button in the "Action" column. While it is necessary to place links to external stylesheets in the head for standards compliance, the same is not true for javascript, because a script can be placed anywhere in the HTML code. Inline scripting is, after all, part of the HTML specification.

It seems you are trying to load a 0.6 template into a 0.7 shop. Actually, it's the other way around - ie. trying to apply a 0.7.7 template to a 0.6.5 setup. I have some templates available for 0.6.5.
