New shipping module like Citylink

Like Citylink shipping module. I am looking to create a new module under shipping.

Fitting request:
No Fitting - No Fee
Easy: $20
Medium: $30
Complex: $50

I would like to make a module so the customer can choose
1. Shipping
2. Assembly request: With 4 Radio options to choose and this value added based on customer choice.

Let me know the details how it can be done? Either I need to clone Citylink shipping? We have more options ?

You can use the built-in Weight Based Shipping method for this. You'd create multiple geo zones for each of your shipping options, and then set those options up in the extension. For more info, see my tutorial here:
