Category Based Order Form - Module

Hello All,

I am looking for someone to create a module that allows all products within the selected category to be added to an order form.

The order form needs to be able to list the product name with a link to the product and a quantity field.
I would prefer to have one add to cart button at the bottom of the form but understand if that is not possible.

Here is the example The idea is that the products in the category are essentially the same except for the package size and price but it is necessary for me to have the products each as individual products as well.

Have to be able to select the categories to have it enabled for as it would not be universal.

Would prefer an oc or vqmod as opposed to altering the core. Please let me know an estimate number of hours to complete as well as your cost per hour.

Thank you for that! I've researched those and two of them might work but they are incompatible with my current version. I've contacted the developers to see if they are able to update them to work with 2.2.0.0. In the mean time I am still accepting quotes for this project.
