Contact form extra fields

DESCRIPTION

- This vqmod will add a new contact fields to your Contact page.
- Fields will be displayed after the Email field.
- It supports multi-language, displaying translated labels for each of your languages.
- You can rename the fields using the XML file, using the corresponding language entries.

FIELDS ADDED

- Item Name
- Item URL
- Phone number
- Ship address
- Remember, any of those 4 fields can be deleter or re-named, using the XML.
