This extension is used for to create custom contact us form. We can manage fields from admin also activate to display some already created fields with custom Address and Map(iframe) field.

Following are the list of files that you need to upload to your live server:
• admin/language/english/module/contact.php
• admin/controller/module/contact.php
• admin/view/template/module/contact.tpl
• catalog/controller/module/contact.php
• catalog/language/english/module/contact.php
• catalog/view/theme/default/template/module/contact.tpl

=> To activate go to Extensions->Modules->Contact Us(Install) then Edit and Activate to display on the related page.
