Questionnaire for customer

This is a Questionnaire module for customer, Admin add questions and answers options from admin panel, after that customer Logein and select or enter answer options against every question after clicking the Questionnaire link.
Then system automatically send email to admin and customer also.
Please just replace the xml file.
