This extension will create "Wholesale form" page with fields given in the above images. From

where customers can fill out fields and admin will recieve an email with detailed information.

All you need to do, is to install the extension, and put link of this page where you want.

You will get an email with customer's name, email, message and product name which they want to

know about and all other information.

Just see the pictures, and you'll get the idea ;)

Installation is as simple as it can be, just upload the files you have downloaded and create a

link where you want. it's all done. No files are overwritten, no complicated database queries, no

trouble at all. In theme folder, add file to your specific theme. Currently i have created default folder. User your theme to add file.
