This extension replaces the images links on storefront from your domain to amazon s3 bucket based.
So there is no risk, if you just upload all your existing images to amazon s3 bucket successfully. So copy your all current images to s3 and verify a few products, if the files uploads are successful , then your store is not going to break at all.

When you update any product , change its images and set new images. The product if opened on store front, the cached images are generated and uploaded to s3, if due to any reason caching fails, the original image will be used as fallback.

So its pretty much secure and less risky to install on an already running store with a lot of products. Just upload your existing and that's all you need to make sure the store doesn't have image broken.
